When you contact our office, you’ll be asked what services you're interested in and directed to fill out the appropriate new patient forms. This will provide important information about you and your specific health condition and goals. Once you have completed your intake forms, we will do a thorough inventory of your health, including lifestyle habits, your medical history, and previous care received.
Please arrive on time for your appointment. Lateness causes delays in the schedule and will limit your time with the Doctors. Lateness exceeding 15 minutes may result in a forced cancellation. To err on the side of caution, arrive 15 minutes before your scheduled appointment time. Plan to spend approximately 30-60 minutes total in the office.
Please email or bring any recent and relevant laboratory testing or any previous diagnostic documents pertaining to your symptoms. Initial appointments are 45 minutes and follow up appointments are 15-45 minutes. Subsequent appointments vary for each person and are only scheduled if your treatment plan requires a follow up. There is not one single cookie cutter plan, Dr. Lazarus will create a personalized treatment plan after he has discussed with you. He provides numerous clinical nutrition protocols based on your condition or goals.
This initial evaluation will last 45 minutes and include a Functional Matrix based evaluation. This includes a through history and evaluation of your symptoms, lifestyle and specific goals. Dr. Lazarus will gather information regarding your environment influences on genetic expression, nutritional influences on genetic expression (nutrigenetics), nutritional influences on metabolic functions (nutrigenomics), metabolic derangements resulting is system failures (metabolomics), psychological and mind body influences on systems biology functions and genetics, and if applicable a personalized genetic evaluation.
Once this evaluation is complete, a personalized nutraceutical, dietary and if indicated an evidence based herbal regimen will be created for the patient. The above will be in accordance with principles outlined through the American Board of Integrative and Holistic Medicine, The American Board of Physician Specialties and The Institute for Functional Medicine.
Physical, neurological, orthopedic examinations will be performed, and your spinal and muscular condition will be assessed in order to discover conditions that may be affecting your health. We may order laboratory tests, x-rays, and other diagnostic images and procedures as required. Based on the information gathered from the initial visit, we will complete your total health profile. Once the complete picture has come into focus, we will assign priorities to your health issues and design your individual course of treatment.
Once we have identified the cause of your health problem, we will begin care by administering adjustments and/or soft-tissue therapies. An “adjustment,” as doctors of chiropractic use the term, is a correction of any joint in the body which has abnormal movement patterns or fails to function normally. The adjustment can be given by hand or instrument and consists of applying pressure to the joint areas that are out of alignment or that do not move properly within their normal range of motion. We will re-evaluate you at frequent regular intervals to ensure that your care remains on target and your health goals are being achieved.
